CCBHC Case Manager
Responsibilities include: Direct Clinical Service - Provides mental health case/care management services to adults, children and families; provides collaborative care planning, referral and monitoring of treatment. Coordinates services needed within the agency and within the community. Communicates with other service providers to improve quality of care to patient as well as quality outcomes. May provide transport for clients to services; promotes a client/driven, strength-oriented service; adheres to the agency's mission statement; policy and procedures; follows guidelines to ensure quality assurance; maintains productivity and documentation standards. Manifest a commitment to and Recovery Model philosophy and standards, and foster an environment that supports recovery for persons served. Possess a working knowledge of community resources. Non-billable, assertive outreach to clients not engaging and may include telephone calls or home visits. Documentation - Completes all appropriate case documentation and case planning information; appropriate documentation of collateral contacts; participates in clinical supervision. Performs related office/administrative duties, such as scheduling patients, answering phones, assist in facilitating telehealth appointments; assist staff in opening and closing of cases; attends staff meetings, supervisions and trainings. Qualifications: Ohio driver's license; proof of automobile liability insurance (minimum $100,000); Bachelor’s degree required. Preference given to LSW/LISW/LPC/LPCC/MFT licensure. Experience in working in the mental health field; exhibits respect, compassion, warmth, caring and friendliness; non-judgmental of varying cultural beliefs; ability to assess, teach and model skill development techniques; ability to set limits, confront behaviors and redirect.
