CCBHC Nurse Practitioner
Well established behavioral healthcare agency seeking a Family Certified Nurse Practitioner to join our new Certified Community Behavioral Health Care Clinic (CCBHC). *Eligible for $5000 sign on bonus*
Responsibilities include: Collaborating with mental health professionals in an outpatient setting, regarding patient care as well as providing direct patient care. Conduct ill visits, preventative medicine visits, well women exams, new patient, and routine check-ups. Collection of and reporting on BMI, weight assessment and nutrition/physical activity assessment for children and adolescents, care for controlling high blood pressure, diabetes screening for people using antipsychotics, diabetes care for people with serious mental illness, metabolic monitoring, and cardiovascular health screening for people on antipsychotics.
Under collaboration with primary care physician, consultation in the comprehensive person-centered and family-centered diagnostic and treatment planning process with the CCBHC treatment team which includes medical director, primary care physician, psychiatrists, nurse practitioners, nurses, counselors, social workers, peer supporters, and case managers.
Provision of both acute and chronic health care services with the collaboration of CCBHC clinicians ensures that patients receive preventive care and screening, educating, and monitoring clients, collaborating with treatment team in conjunction with nursing and behavioral health staff. Review labs, ancillary tests, studies, and consultations from specialists and follow up.

Qualifications: Masters of Science in Nursing (MSN) as Family Nurse Practitioner. Active license, AANP or ANCC certification and credentialing in Ohio. Position requires strong interpersonal communication skills; knowledge of the recovery model; ability to work effectively with interdisciplinary teams; ability to address the cultural needs of each client and family member; ability to work with patients of all ages; and supervisory and leadership skills a plus.
