Responsibilities include: Support Function- Provides on-going engagement and interaction with new clients for intake to assist through the process and support; assists in facilitating linkage and engagement with assigned clinicians; provides ongoing engagement and interventions to existing SMD clients who would benefit from the Peer Supporters own lived experience of recovery; provides linkage, coordination, monitoring and outreach functions and perform documentation requirements both in the community with clients and over the phone; supports engagement of clients in services by assisting case management staff with outreach visits in the community Supervisor: CCBHC Program Coordinator
