Child Clinical Community Support (CCS) Specialist
Responsibilities include: Provides direct case management services for children and their families; coordinates care; coordinates after care planning; monitors client progress; evaluates client services; participates in external staffings or team meetings; provides or assists in crisis intervention & stabilization; offers advocacy services; co-facilitates therapy groups; provides clients with assistance in daily living. Facilitates case management groups. Completes all appropriate service documentation and case management planning; consults with agency therapists; participates in clinical supervision & Continuous Quality Plan process; travels to client's site; completes progress notes, case management plan and other required documentation; obtains appropriate releases of information.
Qualifications: Associate's Degree or greater in Social Work, Psychology or closely related field preferred. Minimum two years experience in community mental health or closely related human service field preferred. Ohio driver's license; proof of automobile liability insurance (minimum $100,000). Position requires experience, skill, comfort with children and families; knowledge of community resources; ability to function as a treatment team member; an understanding of family dynamics and family preservation principles; understanding the Recovery model and working in this model with families; an ability to exhibit and convey respect, warmth, caring and a sense of hopefulness; strong interpersonal skills; and sensitivity to the cultural needs of each child and family.
