Psychiatric Services Case Manager
Responsibilities include assisting nursing staff and prescribers; duties include scheduling, coordinating telehealth appointments, coordination of other agency program services, drug screenings, records gathering and storage, and community support services as indicated. Provides on-going primary and secondary community services to clients, serving as an advocate, liaison, mediator, broker for clients. Completes all appropriate case documentation and case planning information; maintains documentation standards.
Qualifications: Associate degree or greater in Social Work, Psychology or closely related field preferred. Minimum two years’ experience in community mental health or closely related human service field preferred. Ohio driver's license; proof of automobile liability insurance (minimum $100,000).
