SUD Nurse - LPN
Responsibilities: Performing duties that include completing basic medical/nursing assessments and mental status assessments.
Verify any medication changes, recent hospitalizations, or changes in medical, social, or family history that may have an impact on Mental Health Treatment.
Complete necessary charting that is required to maintain standards established to ensure compliance. Communicate Medically Assisted Treatment (MAT) Program procedures and requirements to the client. Collect urine drug screen (UDS), including observed urine drug screens.
Assess clients for side effects of prescribed medications at time of each visit. Document findings in client records.
Provide education regarding medication compliance and potential adverse reactions to client.
Document in clients' records all medications delivered to client or significant other noting delivery made as ordered by physician.
Qualifications: Licensed Practical Nurse (LPN) in the State of Ohio. Experienced in or knowledge of mental health interventions and ability to assess mental illness.
Attendance and participation in continuing education programs designed to maintain an unencumbered RN/LPN license and nursing proficiency.
Position requires an ability to work effectively as part of an interdisciplinary agency team, to communicate and relate effectively with individuals of all ages and families, and to be sensitive and attentive to the cultural needs of each client and family.
Position also requires good computer literacy skills, written communication skills, and strong organizational skills.
