About the Opportunity

We have just increased our salary schedule, and are currently seeking full-time Case Managers in Lancaster, Ohio, for our Supported Employment Team, Crisis Team, Substance Abuse Team, and Outreach and Engagement Team.


What will you do?

Responsibilities for this position include:

  • Providing on-going primary and secondary community services to adults who have Severe Mental Disorders (SMD’s).

  • Serve as advocate, liaison, mediator, and broker for adults with SMD’s.

  • Engage in outreach to clients/families.

  • Attending meetings and coordinating care, including communication with other providers, and discharge planning with hospitals.

  • Develop and implement initial and revised Individual Service Plans.

  • Provide transportation services, and assist with finding transportation services.



What kind of qualifications do you need?

Do you have experience working with adults who have Severe Mental Disorders, a valid Ohio Driver’s license, along with automobile liability insurance (minimum $100,000)? Then this position might be for you!

Applicants for this role must also be able to provide non-judgmental services to people with a wide variety of experience, cultures, and beliefs, a commitment to Recovery Model philosophy and standards, and the ability to foster an environment to support the recovery for the people we serve. Additionally, applicants must be able to teach and model skills, set limits, confront behaviors, and provide appropriate redirection.
